Dementia Care Aware empowers primary care providers and their teams to screen for dementia. Take our training on a 5-10-minute cognitive screening tool called the cognitive health assessment (CHA). This tool can be used annually or whenever a cognitive screen is indicated, such as during Medicare Annual Wellness Visit. 

The Alzheimer’s Project aims to address the devastating toll of Alzheimer’s disease and related dementias in San Diego County. It brings together teams of experts and decision-makers that include members of our region’s top political leadership, research institutions, public universities, health care systems, caregiver groups, and other community organizations.

Our center conducts research studies dedicated to understanding the causes, clinical features, and treatments for Alzheimer’s disease (AD) and related memory disorders.

Alzheimer’s can feel like an impossible disease. But at Alzheimer’s San Diego, we know that people can still live well after a diagnosis. We recognize the dignity of every person living with dementia, and empower them to play an active role in planning their own future care.

The Alzheimer’s Association leads the way to end Alzheimer’s and all other dementia — by accelerating global research, driving risk reduction and early detection, and maximizing quality care and support.

Our mission is to provide affordable and accessible adult day care and support services to families affected by Alzheimer’s and other forms of memory impairment diseases. We do this by providing innovative adult day care programs, family support, case management, crisis intervention, family and community education, advocacy, information and referrals.

The mission of SCRC is to help families and communities master the challenges of caring for adults with chronic and disabling conditions.
